How do we make a support book (a "user's guide" to our child)? What should we include?

It's a self-made booklet to tell a new teacher or support person about your child. Fit onto one or two pages: (1) things they like and are good at, (2) things they dislike and triggers for meltdowns, (3) ways they calm down, (4) phrasing that gets through easily and phrasing that doesn't, and (5) how they communicate (speech, signs, cards). The trick is to focus on "do it this way and it goes well" rather than "problem behaviors." Updating it and handing it over each time your child moves up a grade, changes schools, or starts with a new support provider makes handing off support much easier.
